Applying for the burial subsidy of parents and spouse should attach the Household Certificate Copy from May 1st 2006

According to Article 88 of the Enforcement Rules of the Labor Insurance Act, the insured person applying for burial subsidy should attach the following documents according to Article 62 of the Act: 1, The burial subsidy application form. 2, The payment receipt . 3. The medical certificate of the death or autopsy report issued by the prosecutor, the death must be confirmed by the court verdict. 4. The Household Certificate Transcription of all family members that records the death date of the insured person and the adoption date as well as registration date if the deceased was the adopted child. To match the policy of reducing the quantity of the Household Certificate Transcription, the Bureau of Labor Insurance have consulted with the Department of Household Registration Affair,the Ministry of the Internal on the acquiring of the household register data through computer transmission file. From May 1st 2006, the insured person of the Labor Insurance applying for the burial subsidy of parents or spouse, besides filling in the application form, attaching it with the death certificate according to the above mentioned enforcement rule, needs only to attach the Household Certificate Copy that records the death date of the deceased, together with the ID card or Household Certificate Copy of the insured person, and needs no longer to apply for the Household Certificate Transcription from the Household Registration Affairs office. However, in case the deceased is the insured person's child, the Household Certificate Transcription that records the death date of the deceased should still be attached, as the above mentioned computer transmission file does not include the relationship certificate column; the Transcription should record the adoption date as well as registration date if the deceased is the adopted child, and If the computer transmission file does not provide accurate and complete household data, the Bureau of Labor Insurance will inform the applicant by mail to amend by providing the Household Certificate Transcription. As to the beneficiary claiming for the survivor's benefit of the insured person, the Household Certificate Transcriptions of the insured person as well the beneficiary should still be attached.
