2. How should I apply for the labor insurance health examination for occupational diseases prevention?

  1. You may apply for this examination any time of the year. The insured unit itself may apply for the examination for the eligible insured persons who work for it in batches according to actual situations. The application can be submitted in the following ways:

    (1)Application through the Internet: The application can be submitted through the e-service system of the BLI, and the work environment monitoring results should be uploaded to the Laborers’ work environment monitoring and hazards exposure management internet login system additionally.
    (2) Application by means of a Disk/CD: You may fill out the Application Form for Labor Insurance Health Examination for Occupational Diseases Prevention and submit a Disk/CD with insured persons-related data stored on it, and upload the work environment monitoring results to the Laborers’ work environment monitoring and hazards exposure management internet login system additionally.
    (3) Paper application (if the number of applicants is more than 50, the application shall be done through the Internet or Disk/CD): Please fill out the Application Form for Labor Insurance Health Examination for Occupational Diseases Prevention and submit it together with a roll of applicants for the labor insurance health examination for occupational diseases prevention and upload the work environment monitoring results to the Laborers’ work environment monitoring and hazards exposure management internet login system additionally.
    (4) Application submitted by the insured person himself/herself (in case the insured unit does not apply in accordance with the regulations): Please fill out the Insured Person’s Application Form for Labor Insurance Health Examination for Occupational Diseases Prevention and submit it together with an original copy of working environment evaluation report. (The photocopy document shall bear the seal of the insured unit and its person in charge.)

  2. As for those insured units that are not required to conduct working environment monitoring by law, please provide other relevant certification or such detailed information as the workers’ job descriptions and the location and time of the operation. If the insured person belongs to a occupational union, please also fill out a Report on Working Experiences with Special Health Hazards for Labor Insurance Health Examination for Occupational Diseases Prevention (applicable to the insured person of an occupational union) or a Guarantee Letter of Working Experiences with Special Health Hazards for Labor Insurance Health Examination for Occupational Diseases Prevention. (applicable to the insured person of an occupational union)

  3. After receiving and reviewing the application forms, the BLI will deliver to the insured unit a List of Designated Hospitals for Labor Insurance Health Examination for Occupational Diseases Prevention and one copy of the Certificate & Record of Labor Insurance Health Examination for Occupational Diseases Prevention for each eligible insured person, advising it to download, according to the approved categories of examination, the Worker’s Special Physical Conditions and Health Examination Record from the website of the BLI at www.bli.gov.tw (path: labor insurance/insurance benefits/health examination for occupational diseases prevention/worker’s special physical condition and health examination record). And the insured unit shall in turn advise the insured persons to receive the health examination at the designated medical institutions before March 31th next year.
