5. What should an insured person pay attention to while going to an approved medical institution to receive the health examination?

  1. Before an insured person go to an approved medical institution to receive the examination, he/she shall properly fill out the Worker’s Special Physical Conditions and Health Examination Record in advance. And while receiving the examination, the insured person shall present the health examination certificate & record issued by the BLI, the Worker’s Special Physical Conditions and Health Examination Record and his/her national identity card or any other document that can prove his/her identity.
     
  2. Before the examination, the insured person shall go to the website of the Occupational Safety and Health Administration to find out the list of approved medical institutions for health examination of workers with special physical conditions, choose an appropriate medical institution and contact it in advance to facilitate its operation and the examination.
     
  3. The insured unit shall not ask any insured person to receive the examination in an approved medical institution until the BLI approves the application and issues the Certificate & Record of Labor Insurance Health Examination for Occupational Diseases Prevention. If the examination is received before the said health examination certificate and record is issued, the fees will not be paid by the BLI.
