12. Is a female insured person eligible for claiming disability benefits after having hysterectomy due to uterine fibroids at more than 45 years of age?

If a female insured person originally has fertility and is less than 45 years of age, but has her ovary or uterus of both sides cut and lead to the loss of reproductive function due to injuries or sicknesses during effective period of the insurance coverage, she is eligible for claiming disability benefits by sending the Disability Diagnosis Report issued by National Health Insurance special contracted hospitals or clinics to the Bureau of Labor Insurance directly. She should also enclose the Labor Insurance Disability Application Form and Payment Receipts and the “Certificate of Proof for sending Labor Insurance Disability Diagnosis Report directly to Bureau of Labor Insurance”. However, if the insured person is more than 45 years of age when having the hysterectomy, she is not eligible for applying for disability benefits.
