2. What are the documents needed for claiming survivor benefits?

  1. Funeral grant:

    (1) Application forms for Survivor Benefit and Receipts of payments.

    (2) Death certificates, written autopsy reports issued by the public prosecutors, or judgments of proclamation of death of a missing person.

    (3) The certificate of household registration for the whole household with the dates of death recorded and the benefit applicant's current address household registration certificate which is applied and issued after the day the insured persons die.( Do not omit the notes column)

    (4) Receipts or evidencing documents for funeral expenses. However, if the person who paid the funeral expenses is also the first in line to receive a survivor pension or benefits, an affidavit may be used as a substitute.
     
  2. Survivor allowance or survivor pension:

    (1) Application forms for Survivor Benefit and Receipts of payments.

    (2) Death certificates, written autopsy reports issued by the public prosecutors, or judgments of proclamation of death of a missing person.

    (3) The certificate of household registration for the whole household with the dates of death recorded and the benefit applicant's current address household registration certificate which is applied and issued after the day the insured persons die.( Do not omit the notes column)

    (4) If the dependents are minor or have no capacity to make juridical acts, the application form should contain the affixed seal of the legal representative of the dependents and be enclosed with household registration certificate of the legal representative.

    (5) If the dependents are grandchildren, brothers or sisters of the insured person, the related proof of being brought up by the insured person shall be enclosed.

    (6) In case of the death of a foreign worker, or the dependents do not have household registration in Taiwan, please refer to the explanation of no. 5. The certificates and related proofs shall be re-submitted to the BLI for checking every year for survivor pension applicant.

    (7) Other documents which should be enclosed for claiming survivor pensions:
    a、 If the dependent is a spouse, the marriage date shall be recorded on the household registration certificates.
    b、 If the dependents are attending schools, the copy of Tuition Receipt or In-school certificate shall be enclosed, and those certificates and related proof shall be re-submitted to the insurer for checking every year before the end of September. If the information complies with the criteria after checking, the Allowance would continue to be issued until the end of August of the next year.
    c、 For dependents who are unable to make their own living, a physical and mental disability manual or related proof or any proof of the dependents having been declared by a Court of law to be unable to manage their own assets shall be enclosed.
