9. What documents should the dependent of a deceased labor or the designated person for claiming the pension in the will of the deceased labor prepare for claiming the pension?
- Fill out a “Labor Pension Application Form and Receipt by Worker’s Dependent or Designated Applicant.”
- Transcript of household registration for the whole household with the date of death, or death certificate, or autopsy certificate issued by a prosecutor, or Death Declaration Judgment.
- If the applicant and the deceased labor are not under the same household registration, the applicant’s transcript of household registration applied after the deceased labor’s date of death should be enclosed.
- Photocopy of the will (it should be enclosed when the applicant is designated applicant in the deceased worker’s will, Please also make a note that it is the same with the original copy and then sign or affix the applicant’s seal on the copy.
- If the applicant is a foreigner (or a resident of the Mainland China), please provide a copy of the personal identity document.
