2. How can the occupational injury/sickness medical treatment forms be obtained?

When the insured suffers occupational injury or disease requiring hospitalization or outpatient treatment, the insured unit or the insured may download the occupational injury and disease outpatient clinic form and hospitalization application form from the official site of the Bureau. After the insured unit has stamped and certified the documents, the insured may submit such documents to the NHI hospital or clinic for further treatment. If the insured unit fails to fill in the form according to the regulations, the insured may make a claim to the Bureau and local offices and the benefits will be distributed after verification.
