1. What are the eligibility requirements, payment criteria and required documents for permanent disability allowances for Occupational Accident Hospitalized Treatment?
(1) Qualification:
Those who suffer a permanent disability due to occupational diseases whose degree of disability conforms to the provisions of the disability benefit standard of labor occupational accident insurance.
(2) Benefit Standard:
①The benefit standard of Permanent disability allowances is basing on the average monthly insured salary when the insured withdraws from the insurance. If the average monthly insured salary calculated is lower than the amount of Level 1 of the insurance salary scale of the Labor Occupational Accident Insurance and Protection Act at the time of diagnosis of permanent disability or death, it shall be calculated and paid according to the amount of Level 1.
② The number of days of the disability level approved according to the permanent disability benefits distribution criteria of labor occupational accident insurance. The highest disability level is the first level with the highest lump-sum disability benefits of 1,800 days and the lowest level is the fifteenth level with the lowest lump-sum disability benefits of 45 days. Those who apply for permanent disability allowances may only apply once.
(3) Required documents:
①Surrender Occupational Disease Permanent Disability Allowance Application Form and Payment Receipt of the distribution
② Labor Occupational Accident Insurance Permanent disability Statement.
③Those who have undergone medical examination shall submit the examination report and relevant images.
④Occupational illness diagnosis certificate.
⑤Labor Occupational Accident Insurance Occupational Sickness Work Resume(exempted from submission if the content of the working record has been specified in details in the occupational disease diagnosis certificate).
