10. How do I request an additional copy of the payment slip if I haven't received it?
If the insured unit has not received the payment slip by the end of the month, please request an additional copy from the Bureau of Labor Insurance as soon as possible to avoid overdue payments. You may obtain a copy of the payment slip via any of the following four methods:
- Via the Internet :
Visit the
Bureau of Labor Insurance Chinese website. Click on the "Premiums/Labor Pension Query and Payment Slip Request", "Labor Insurance and Employment Insurance Premiums Query and Payment Slip Request" under "Internet e-Counter" at the upper left corner. Then enter the insurance ID of your unit and the validation code to generate PDF file, and you can print out the payment slip on your printer.
- By telephone:
Call the Bureau of Labor Insurance at (02) 2396-1266 Ext. 3111 and explain that you have not received the payment slip in the mail. Inform the Bureau service representative of your Insurance ID, Name of Insured Unit and the year and month of the payment slip you are requesting. The Bureau will promptly send you a copy of the payment slip via registered mail.
- Written application:
Please specify your Insurance ID, Name of Insured Unit and the year and month of the payment slip you would like to receive in a written request. After the Bureau has received your request, will promptly send you a copy of the payment slip via registered mail.
- Apply in person to the Bureau of Labor Insurance or any local office:
Make a request by visiting the Bureau of Labor Insurance or the Single Service Window of any of our local offices. You will be reissued a copy of the payment slip promptly.
