If the insured suffers any occupational injury or disease during the effective period of the insurance, the symptoms are fixed following treatment, further treatment will not improve the result of treatment, them within effective insurance coverage period or 1 year after withdrawing from the insurance, the person is diagnosed with permanent disability by a contracted hospital or clinic of national health insurance, meeting the conditions for Permanent Disability Benefits under labor occupational accident insurance, the insured may, within a 5-year period of claim, contact any NHI hospital or clinic that is one of the "medical centers or regional hospitals verified by the Ministry of Health and Welfare and passed the hospital evaluation and teaching hospital evaluation" to issue the Labor Occupational Accident Insurance Permanent disability Statement to be directly submitted to the Bureau by such hospital or clinic, and shall fill in the Labor Occupational Accident Insurance Disability Benefit Application Form and benefit receipt, which will be along with the “Certificate for Direct Submission of the Labor and Occupational Accident Insurance Permanent Disability Diagnosis Certificate” submitted to the Bureau who then issue the relevant benefits after comprehensively reviewing the applicable disability level and number of days for the insured in accordance with the above-mentioned regulations.
