18. Following the induction of the individualized professional assessment mechanism, has the disability benefits application method been changed? How does one apply for the individualized professional assessment?

There is no difference, but an individual is reminded to fill out and submit the new version of the disability benefits application form.

In response to the induction of the individualized professional assessment mechanism, the Bureau of Labor Insurance has since amended the new edition (August 2013) of the “Labor Insurance Disability Benefits Application and Payment Receipts”, which states relevant regulations, and is intended to avoid undermining the insurance equity, and when filing for the disability benefits at a later date, an individual is kindly asked to fill out and submit the new version of the benefits application, and for those wanting to file for the “pension”, please check the “collecting the disability benefits payout by month”, and since it cannot be changed once the payout is approved and released, please ensure that you consider stringently and select carefully.

The individualized professional assessment does not require the insured person to file for a separate application, as long as the insured checks the “collecting the disability benefits payout by month” in the benefits application, and which upon reviewed by the Bureau of Labor Insurance, of those needing to undergo the individualized professional assessment, the Bureau of Labor Insurance will gather relevant information forwarding for professional hospital review, and the evaluation finding is only used by the Bureau of Labor Insurance for reviewing the disability pension payout, and may not be applied for any other purposes.
