3. To apply for maternity benefits, what documents should I prepare?

  1. Maternity Benefits Application Form and Payment Receipt. (Individual insured persons can file their own applications. If you apply by yourself, leave the “Insured Unit Endorsement” column blank on the Labor Insurance Maternity Benefits Application Form and Payment Receipt.)
     
  2. Original birth certificate of the infant. (If the birth has been registered, the certificate may be exempted.)
     
  3. For those who apply with a foreign birth certificate, except enclosing the photocopy of the insured persons’ passport, they shall also follow the regulations below:
    (1) If the foreign birth certificate is made abroad, it should be certified by a ROC embassy, representative offices, liaison office or any other organization authorized by the Ministry of Foreign Affairs (the diplomatic agencies); If the foreign birth certificate is issued by a foreign ambassador agencies in Taiwan or authorized institutions, it should be verified and certified by the Ministry of Foreign Affairs.
    (2) If the certificate is completed and issued in Mainland China area, it should be certified by a notary public office of the People’s Republic of China and then authenticated by the Straits Exchange Foundation.
    (3) If the certificate is completed and issued in Hong Kong or Macau, it should be certified by the Taipei Economic and Cultural Office in Hong Kong or Macau.
    (4) Documents that are written in a foreign language should also include a Chinese translation under the certification of the agencies listed above or the notary in Taiwan. Unless it is considered necessary by the insurer, the Chinese translation is not required if the document is made in English.
     
  4. In the event of miscarriage, please enclose the certificates of miscarriage prepared and issued by the hospitals and clinics under the National Health Insurance program, or licensed doctors and midwives. (The certificate shall record data such as the serial number of National Identification Card, date of birth, name, actual date and reason of miscarriage and the last period date of the pregnant woman)
